Abstract： Intertransverse process block （ITPB）guided byultrasound isa type of paraspinal nerve block technique.Its coreoperationis toinjectlocalanestheticsintotheintertransversetissuecomplexbehindtheribtransverseprocesligament underreal-timeultrasound guidance.Due toitssimpleoperation，safetyandeffectiveness，andfewcomplications，ithas atractedattentionandisconsidered the mostpromisingalternative toparaspinal nerve block.Atpresent，theapplication of ultrasound-guided ITPB in clinical practiceisincreasing，especially inthoracic，breastandother surgeries，showing significantanalgesicefectsandhelpingpatientsrecoverquicklyaftersurgery.Thisarticlereviewstheanatomicalbasisof ITPB，theoperation methodsanddrug selectionofultrasound-guidedITPB，perioperativeclinicalapplications，andpotential complications basedonrecent domestic and foreign research literature，inorder to providea reference forthesafe and effective application of this technology in clinical practice.
