Abstract：Theglobalriseindiabetes prevalencehasledtoanincreasedriskofdiabeticnephropathy（DN），amajorlethal microvascular complication characterized by complex pathogenesis.Although current therapies aleviate symptoms through glycemiccontrolandalbuminuriareduction，theyfail tohaltdiseaseprogression，withmostpatientseventually developingend-stagerenal disease.Recentstudieshighlightthetherapeutic potential of extracellar vesicles（EVs）in DN management.Functioning as intercellularcommunicators，EVscarry bioactivecomponentssuchas nucleicacids and proteinsthatcriticallyregulatepathological processes.Thisreviewsystematicallyexaminesemergingmechanismsby whichEVsmitigaterenalfibrosisandsuppress inflammatoryresponses，whileevaluatingtheirclinicaltranslationprospects fordeveloping targeted therapeutic strategies against DN.
