老年甲状腺乳头状癌超声表现与BRAF基因突变及临床病理特征的相关性

[Abstract] ObjectiveTo analyze the correlation between ultrasonographic findings of papillary thyroid carcinoma and BRAFgenemutationandclinicopathologicalfeaturesintheelderly.MethodsAretrospectiveanalysiswasconductedon1lelderly patientswithpapllarythyroidcarcinomaadmittedtotheAflitedCancerHospitalofNantongUniversityfromMarch22toApril 2024.Allpatientsunderwentultrasoundexamination，andpolymerasechainreactiontechnologywasusedtodetecttheBRAFgene. TheclinicopathologicalchracteristcsandurasoundmanifestatiosoftepatientswerecolectedAccordingtoeresultsofBAF gene detection，the patients were divided into BRAF mutation （ + ）（mutant group）and BRAF mutation（-）（wild type group）. MultivariatelogisticregesionanalysiswasusedtoanalzetheindependentpredictivefactorsofBRAFgene mutation.ResultsThe BRAF genetestresultsshowedthat therewere8Ocasesinthe mutantgroupand41cases inthewild-typegroup.Theabnoal thyroid globulinantibody（TG-Ab），extracapsularinvasion，lymphnode metastasisinthecentralcervicalregion，combinedbenignnodules，verticallesionlocaorlararinndeproortioofcrocalifatioiutantgoupeeallhighranoseieildtype group （ P<0.05 ），and the maximum diameter of the tumor was longer than that in the wild-type group （ P<0.05 ）. There were no stiticallysignifcantdiferencesbetweehetogroupsitersofgnde，age，abnoalthyroidstimulatingormon，aboal thyroglobulinboalthodproxidaseaodymberofsios，ombindHasimoto’syrodis，esionlocation，ohc halo，solid component echo，thyroid background，and blood flow Rago classification（ P>0.05 ）.Multivariate logistic regression analysisshowedthatTG-Ababnormality，extracapsularinvasin，maximum tumordiameter，centralcervicallymphnodemetastasis，and vertical positionofthelesionwereindependentpredictorsofBRAFmutation（ P<0.05 ）.ConclusionThecharacteristicssuch asabnormalTG-Ab，extracapsularinvasion，themaximumdiameterofthetumor，lymphodeetastasisinthecentralcervicalgioand vertical lesion in elderly patients with papillary thyroid carcinomaare closely related to BRAF gene mutations.
