[Abstract]Priapismrefers toaconditioninwhich thepenisremainserectandcannot become flaccidafter sexual excitementororgasm，resultinginprolongedandpainfulerections.Accordingtoitsclinicalmanifestations，ancientandodern practitionersofinesemedicinelasifyprapismunderategoresschs"nisgidity，"prolongedpenisretio，nd"sistet erection".Asthebasicstructurethatregulatesthefilinganddrainageofqiandbloodinthepenis，"mysteriousmansion （a colectie trmfortheminutestructuresthroughwhichqibloodandbodyfluidscirulateandmove）collterals"partipatesithe wholeprocessofpenileerectionanddetumescene，andisthekeytoensurethenormalerectionofthepenis.Basedonthetheory of"mysteriousmansion-qiandblood-colaterals"，thecorepathogenesisofthisconditionisemphasizedas"disorderof mysteriousmansion，obstructionofcollterals，anddysfunctionofpenis."Itcoversthreepathological stages：exuberantfireithe liverandgalbladercausing the blockageof mysteriousmansionandcolaterals；yindeficiencyandyang hyperactivitycausing mysterious mansiondisharmonyandcolateralmalnutrition;phlegmandbloodstasisobstructingmysteriousmansionandcolaterals. Thetreatmentapproachcenterson"regulating themysteriousmansionandunblocking thecollaterals."According todiferent pathologicalstages，itdrivestheefoundationaltreatmentstrategie：sothingtheliverandreducingfireunblockingthcolateals andactivatingthemysteriousmansion；nourishingyinandreducingfire，moisteningthecolateralsandnourishingthemsterious mansion;eliminatingstasisandresolvingphlegm，unblockingthecolateralsandopeningthemysteriousmansion.Overallthe treatmentemphasizestheprincipleof"aplyingunblockingastheessential"Thispaperaimstoprovideanewperspectiveforthe clinical pattern differentiation and treatment of priapism.
