AbstractObjective：From the perspective ofout-of-hospital extended service，to explore the path of "health manager + PGR system"andtodiscusstheimplementationeffect.Methods：Atotalof75patientsunderwentjointreplacementinourhospitalfrom OctobertoDecember2O23wereselectedasthecontrol group.And72casesunderwentjointreplacementinourhospitalfomJanuaryto February 2O24 were selected as the observation group.The control group was accepted routine "phone + WeChat" follow-up.And the observation group was provided extended nursing service of "health manager + PGR system".Pain Visual Analogue Scale（VAS） scores， DailyLiving Ability（ADL）scores，theWeste OntarioandcMasterUniversitiesOsteoarthritisIndex（WOMAC）scores，Harisip Score（HHS）scores，AmericanShoulderandElbow Surgeons'Form（ASES）scores，postoperativecomplications，patientand nurse satisfactionwerecomparedbetweenthe twogroups.Results：One monthandthree monthsaftersurgery，there weresignificantdifferences in termsofpainVAscores，ADLsores，WOMACscores，HHscoresASESscores，patientsatisfactionandursesatisfaction between the two groups （P<0.05） .Conclusions：The extension service of "health manager + PGR system" could improve patients' compliancearoutsideopitaleleratepidebiliatioprocescortfuctiarlyndimproehalile.
