[Abstract]ObjectiveToanalyze the expressionof serum nuclear factor E2 related factor 2（Nrf2），NOD-likereceptor protein3（NLRP3）inflammasomesandinterleukin-18（IL-18）inelderlypatientswithsepsiscomplicatedwithacute kidneyinjury （AKI）and theirprognosticvalueforpatients.MethodsFromJanuary2O20to December2O23，98elderlypatients withsepsiscomplicatedwithAKIintheGeneralHospitalofTiuanIonandSteel（Group）Co.，Ld.wereseectedstheconcuentgroupandanother60 elderlypatientswithsepsis inthesameperiod were selectedasthecontrol group.SerumNrf2 mRNA，NLRP3mRNAandIL18expresiowerecomparedbetweenthetwogroups；patientsintheconcurentgroupweredividedintosurvivalgroup（72cases）and deathgroup（26cases）accordingto28dsurvival.Multivariatelogisticregresionwasusedtoanalyetheinfluencingfactorsof28d death intheconcurrent group;ROCcurvewasused toanalyze thepredictive valueofserum Nrf2 mRNA，NLRP3 mRNAandIL-18expresionfor28ddeath intheconcurentgroup.ResultsSerum Nrf2 mRNAexpresionwaslowerintheconcurrent groupthaninthe control group （ P<0.05 ），and serum NLRP3 mRNA and IL-18 expression were higher in the concurrent group than in the control group （ P<0.05 ）；multivariate logistic regression analysis showed that elevated APACHEI score，elevated SOFA score，low serum Nif2 mRNA expression，ighNRP3 mRNAandIL-18expressonwereindependentriskfactorsfor28ddeathintecocuentgoup （ P<0.05 ）； ROC curve results showed that the AUC of serum Nrf2 mRNA，NLRP3 mRNA，and IL-18 expression were 0.882， 0.845，andO.865，respectivelyConclusionSerumNrf2mRNA expresion wasdecreased，NLRP3mRNAandIL-18expesion wereincreasedinelderlypatientswithsepsiscomplicatedwithAKI，ndNf2mRNA，NRP3mRNAandIL-18expresioncouldssist 赵亮，等．血清Nrf2、NLRP3及IL-18在老年脓毒症并发急性肾损伤患者中的表达及预后价值 nevaluating the short-term prognosis of elderly patients with sepsis complicated with AKI.
