基于T2WI超分辨率重建影像组学对前列腺癌包膜外侵犯的预测效能

[Abstract]Objective:Toinvestigatethepredictiveeficiencyofsuper-resolutionreconstructionradiomicsforextraprostatic extension(EPE)of prostatecancer.Methods:Atotalof107patientswhounderwentradical prostatectomyforprostatecancer wereenrolled.AllpatientsunderwentMRIscansbeforesurgeryandthenrandomlydividedintoatrainingcohort(74cases) andatestingcohort(33cases)ataratioof7:3.Allimageswerereconstructedwithsuper-resolutionreconstruction.ROI were manually outlined onthe T 2 WI images before and after reconstruction,and radiomics features were extracted.Least absolute shrinkageandselectionoperator(LASSO)regresionwasused toreduce dimensionalityandconstructthemodel. ROCcurvesanddecisioncurves were drawn toevaluate theperformanceandclinicalnet benefitof themodel.DeLong testwasused tocompare theAUC,andtheMcNemar test wasused tocompare theaccuracyand specificity.Results: Postoperative pathology of 1O7 cases confirmed that 40 cases (37.38 % )had EPE.7 radiomics features from image dataset beforereconstructionand6afterreconstructioncloselyrelatedtoEPEwereselected.Andlogisticregresion(LR)clssifier wasusedtoconstructradiomicsmodelsbeforeandafterreconstruction.TheAUCsof themodelbeforereconstructionin thetraining and testing cohorts were 0.819(95%CI 0.708-0.930)and 0.738(95%CI ),while those of the model after reconstruction were 0.852(95%CI 0.757-0.946 )and 0.766(95%CI 0.547-0.985 ),respectively. There was no significant difference in AUC between the two radiomics models( P =0.70),while there were significant difference inaccuracyand specificity(both P <0.05).The decision curve analysis showed that the model after reconstruction had beter clinical net benefits.Conclusions:Super-resolutionreconstructiontechnologycanimprovethepredictiveperformanceforEPEofprostate cancer,which is helpful for clinical preoperative assessment and treatment decision-making.
