肝纤维化血清学指标在慢性乙型肝炎磁共振弹性成像肝纤维化分期中的应用

[Abstract]Objective:Toinvestigatetheapplicationvalueoffourbiomarkersofliverfibrosis,hyaluronicacid(HA),laminin(L), typeIl procollagen(PCII)andtype Vcollagen(CIV),infibrosisstaging inchronic hepatitisB(CHB)bymagnetic resonanceelastography(MRE).Methods:Atotalof242patientswithCHBandliverfibrosiswerecolectedandunderwent serumtestsforthefourbiomarkersandMREexamination.AllpatientswerecategorizedintoliverfibrosisstagesFO,F1,F2, F3andF4basedonMREelasticityvalues.Thecorrelations between thefourbiomarkersandMREelasticityvalues were analyzed,andROCcurveswereusedtoassesstheirdagnosticeficacyforliverfibrosisstaging.Results:HA,LCIl,andCV were positively correlated with MRE elasticity values( ρ =0.513,0.382,0.336,0.563,all P<0.001).With the progression of liver fibrosisstaging,thelevelsofthefourbiomarkersincreasedprogresively.Forthediagnosisofstage≥F1,thediagnostic effcacy of each biomarker was relatively limited,among which the diagnostic efficacyof CN wasrelatively higher(AUCof 0.700).For the diagnosis of stage ⩾ F2,CIV showed a moderate diagnostic efficacy(AUC of O.7o5).For the diagnosis of stage ≥F3,HA and CN had relativelygood diagnosis performance (AUC of O.73O and 0.739,respectively).For the diagnosis ofstageF4,thediagnosticeficacyofeachbiomarkerwasrelativelygood,amongwhichthediagnosticeficacyofHAwasthe highest(AUCofO.873).Conclusions:Thefourbiomarkersofliverfibrosis(HA,LN,PCI,andCIV)arepositivelycorelated withMREelasticityvalues.The biomarkershave certaindiagnosticvalueforliverfibrosisstaging,speciallyHAand CIV. which canprovideauxiliary diagnostic basis for the assessment and stagingof liver fibrosis in CHB patients.
