应用超声心动图及红细胞分布宽度评估乳腺癌患者化疗后左心室功能受损的价值

[Abstract]Objective:Toinvestigatethevalueofusing layer-specificstraintechnique,Teiindex inechocardiographyandred bloodcelldistributionwidth(RDW)toevaluatetheleftventricularfunctionalimpairmentandprognosisofanthracycline(ANT) inbreastcancerpatientsaftersurgery.Methods:Fiftypatientswithbreastcancerwereselected,andaftersurgeryallofthm receivedregularchemotherapywithANTfor4cycles,21daysineachcycle.Layer-specificstrain,TeiindexandRDWwere performedontheday beforethefirstchemotherapy(TO)andthelastdayofeach subsequentchemotherapy(T1,T2,T3,T4). Thevariousparametersfordiferentchemotherapycycleswereobtained,andthechangesofeachparameterindiferent ANT cycleswerecompared.Results:TherewerenosignificantdferencesinLVDd,LVDs,IVSd,LVPWd,LVEForE/e' among different ANT cycles (all P >0.05).GLS-endo,GLS-mid and GLS-epi all showed a downward trend with the increase ofchemotherapycycles,andthechangesofmyocardialstraintendedtobemoreobviousthanthatbeforechemotherapy. GLS-endoandGLS-epibegantodecreasefrom T1,andGLS-midbegantodecreasefromT2.However,bothTeiindexand RDWshowedanincreasingtrend,withTeiindexincreasingfromT1andRDWfromT2.Amongthelayer-specificstrain parameters,Tei index and RDW,the layer-specific strain parameter had the highest Kendall W coefficient,while among thelayer-specific strain parameters,the Kendall W coefficients of GLS-endo and GLS-mid were higher. Conclusions : Echocardiographiclayer-speeificstrain techniquecanconductlocalandpreciseassessmentof leftventricularfunction.TheTei indexcanasess theimpactofANTontheoverallfunctionoftheleftventricle,whileRDWhascertainpredictivevaluefor the prognosis of breast cancer chemotherapy and can guide the adjustment of chemotherapy regimens.
