IVIM模型对直肠腺癌壁外血管侵犯的诊断价值及IVIM各参数值与Ki-67的相关性

[Abstract]Objective:Toexplorethediagnosticvalueoftheintravoxelincoherentmotion(IVIM)modelindetectingextra-mural vascularinvasion(EMVI)inrectaladenocarcinoma,andtoanalyzethecorrelations betweenIVIMparametersandKi-67 expresion.Methods:Atotalof36patientswithpathologicallconfirmedrectaladenocarcinomawereenrolledanddividedinto theEMVIpositivegroup(16cases)andEMVInegativegroup(2Ocases).ThelesionIVIMparameters were measured,such as ADCstandard,ADCslow,ADCfast ,perfusion fraction (f),distribution diffusion coefficient(DDC),diffusion heterogeneity index(α). Thedifferencesineach parameterbetweenthetwogroupswerecompared,ndtheparameterswithstatisticallysignificant diferenceswereincludedintheROCcurvetoevaluateeficacyinthediferentialdiagnosisofEMVInegativeandposie. Spearmancorrelationanalysis wasused to analyze the correlations between parametersand Ki-67level.Results :ADCslow , (20 ADCfast,f and DDC in the EMVI positive group were higher than those in the negative group(all P<0.05 ),whilethere were no significant differences in ADC standard or α between the two groups(both P>0.05 ). For EMVI assessment, ADCfast had the highest AUC(O.918),and when the diagnostic threshold was 5.045×10-3mm2/s,ADCfast had the sensitivity and specificity of O.875and O.943,respectively. Spearman correlation analysis indicated that ADCstandard , ADCslow ,f,DDC and α were all negatively correlated with Ki-67(all P<0.05 ).Conclusions:The IVIM model has a high value in evaluating EMVI of rectal adenocarcinoma,and among all IVIM parameters, ADCfast demonstrates the highest diagnostic accuracy.
