【Abstract】 Objective To observe the changes in gut microbiota in newborns with severe pneumonia and to analyzetheir relationships with poor disease outcomes.MethodsOne hundred and fifty-nine newborns with severe pneumoniaadmited toour hospital from January 2O21 to January 2O23 (observation group)were enrolld,and another159 newborns born inthe hospitaland whose neonatal health information was established(the control group) were selected.Fecalsamples inthe two groups were taken,the gut microbiota were detectedby high-throughput sequencing technology and compared.The observation group received routine treatment and was observed for two years.They were divided into a poor outcome group and a good outcome group based on their conditions.General information and gut microbiota were compared between two groups.Logistic regression analysis was conducted to identify the influencing factors of por disease outcomes in theobservation group.ResultsThe Shannon index of the observation group was lower than thecontrol group,and the relative abundances of Bacteroides,Bifidobacterium, and Lactobacilluswere lower than the control group ( P<0.05 ),ofwhich Enterococcus,Streptococcus,Staphylococcus, Salmonella,and Escherichia were higher than the control group( P <0.05). There were 5 cases of detachment in the observation group,and the incidence of adverse outcomes was 17.53% (27/154).1-minute Apgar score [OR(95%CI)= 0.636(0.422,0.960)],exclusivebreast feeding [OR(95%CI)=0.590(0.389,0.894),probioticadjuvant therapy[OR (95%CI)=0.543(0.363, 0.814)], Shannon index [OR(95%CI)=0.584(0.369,0.925)], Enterococcus [OR(95%CI)=1.527 (20 (1.011,2.304)], Bifidobacterium [OR(95%CI) ==0.583(0.350,0.971)], Staphylococcus ⌊OR(95%CI)=1.493(1.005 , 2.219) were all factors influencing poor disease outcomes in the observation group ( P <0.05).ConclusionThe Shannonindex,Bacteroides,Bifidobacterium,and Lactobacillusdecrease insevereneonatal pneumonia,while Enterococcus,Streptococcus,Staphylococcs,Salmonella,and Escherichia increase,and1-minute Apgarsore, exclusivebreastfeeding,probiotic adjuvant therapy,Shannon index,Enterococcus,Bifidobacteriumand
