【Abstract】 Objective To explore the pathogenic characteristicsand short-term prognostic factors of patientswith malignant tumorsaccompaniedby blood stream infections (BSI).MethodsNinty-eight patients with malignant tumors complicated with BSI who visited from April 2021 to January 2O25 were included in thestudy.The pathogenculture results,drugsensitivitytest results,and clinical dataof the patients werecollected,and theywere divided intoasurvival groupof81cases anda death groupof17 cases based on their3O day treatment outcomes.Univariateand multivariate logistic regresson analysisof risk factors affectingthe short-term prognosis of patients with malignant tumors complicated with BSI.ResultsA total of107 strains of bacteria were isolated from98 patients with malignant tumorscomplicated with BSI,among which Gram negative bacteria were the main ones,accounting for 69.16% ,followed by Gram positive bacteria,accounting for 27.10% .The drug sensitivity results showed that most of the detected Gram positive bacteria were resistant to penicillin andsensitive tovancomycinand ciprofloxacin.Among Gram-negativebacteria,Escherichia coli exhibited highresistance rates to cefazolin,gentamicin,andlevofloxacin,whereas Klebsiell pneumoniae and Pseudomonas aeruginosademonstrated favorablesusceptibilitytomost testedantimicrobial agents.Theresultsofunivariate analysis showed that there was a statistically significant difference ( P <0.05) in clinical data such as age,septic shock,mechanical ventilation,andantibiotic use between the two groups ofpatients.Theresultsof multiple logistic regression analysis showed that the use of antibiotics for≥14 days,prolonged mechanical ventilation time, concomitant septicshock,and granulocytedeficiency for>7dayswere independent factorsaecting the poor prognosis of patients with malignant tumors and BSI ( P <0.05).Conclusion Gram negative bacteria are the main pathogenic bacteriain patients with malignant tumorsandBSI.Theuse of antibioticsfor≥14 days,prolonged mechanical ventilationtime,concomitant septic shock,and granulocyte deficiency time for>7days are independent factors affecting the poor prognosis of patients with malignant tumors and BSI. Corresponding prevention and control measures need to be taken in a timely manner to improve the patient's prognosis.
