Abstract： Objective：To deeply understand the lived experience and needs of patients with end-stage heart failure，and to provide references for beter implementing hospice carefor patients with heartfailure.Methods：A qualitative phenomenological research methodwas adopted to conduct in-depth interviews with15 patients with endstage heart failure.The Colaizi 7-step analysis method wasused to code，analyze，and extract themes from the interview data.Results：Four themes and1Osub-themes were identified：first，experiencing physical and psychological distress （a desire forrelieffromphysical pain and aneed for psychological counseling）；second，ambivalence towards family support （yeaming forcare but feling guilty）；third，activelyseeking social support（expecting tobeunderstood and valued，facilitating accessto support from the medicalsystem，and difering perceptions of doctor-patient shared decision-making）；fourth，complex psychological experience regarding prognosis（experiencing fear and wory，feeling disappointed，living in the moment，and accepting death）.Conclusion：Medical staff，familycaregivers，and society should jointly pay attention tothe physical andmental felings and needsof patients with end-stage heartfailure and provide targeted care.It is recommended to implement multidisciplinary team management，promote doctor-patient shared decision-making，meet individualized needs，and provide appropriate education on life and death concepts， thereby establishing a pallative and hospice care service model for end-stage heart failure with characteristics.
