Three fractionated stereotactic radiotherapy techniques in treatment of intracranial oligometastases:a dosimetric study

【AbstractObectiveToomparethdosimetricracteristicoftrefractionatedereotacticdioterapytehqust therapy(TOMO),volumetric-modulatedarctherapy(VMAT),andCyberKnife(CK),inthetreatmentofintracranialoligometastases, andtoassessthosstrbuio,teametiecynddreceinoseliedtgansats(OARs)ethdst spectiveanalysiswasperforedfortheclinicaldataof54patients withintracranialoligometastaseswhounderwentfractionatedstere tacticradiotherapyinTeFstAfliatedHospitalofAyMedicalUniversityinO-2023.VarianEclipse16.1PhysicianWorkstationWasusedtoperformtumortargetvolumedelieation,andMANTEIAAccContour3.2softwarewasusedtoperformthedelieation ofOARs,uchsistesialodndicals.ThedeateducturesndageserrsitedtoOd Eclips treatment planningsystemstodesign threediferentradiothrapyreatmentplans.Relatedkeyparameters wereanalyzedusing thedose-oluehistogramtoevalatethdosimetriccharacteristicsofthesetrediotherapytechniques,icudingcofoityindex (CI)of thetarget,dosehomogeneityindex(HI),beam-ontime,theumberofmonitorunits(MU),andtheexposuredoseofOARs.
Results:All three treatmentplans(TOMO,VMAT,and CK)met the requirements for prescribed dose.TOMO had a slightly better CI thanVMATandCK(1.05vs.1.09and1.17,P<0.001).VMAThad abetterHI than CKand TOMO(1.15vs.1.28 and1.46,P<0.001). Intermsofexecution efficiency,VMAThadasignificantlyshorter beam-ontimethan TOMOandCK(5minutes,1633MUvs.10 minutes,8 932MU and 39 minutes,5191 MU, P <0.001).In terms of theexposuredoseofOARs,CKprovidedthebestprotectionforthelens,withamaximumdoseof15cGyfortherightlensand17 cGy for the leftlens,and TOMO had an advantage in dose control for therightcochlea,with a mean dose of 88cGy ,whileVMAThad thebestperformanceinlimitingthedoseforthespinalcord,withamaximumdoseof31cGy(P
