Application value of long-term electrocardiogram monitoring with adhesive patch in patientswith palpitation

【Abstract}Obective：Toinvestigate theaplcationvalueof72-our three-leadelectrocardiogram（ECG）monitoring withadesive patch（AP）inpatientswith palpitation.Methods：Aretrospectiveanalysiswasperformedforthe medical historyandECGdataof the patients whounderwent72-our three-leadECG monitoring withAPdue topalpitationinTheFirstAfliaedHospitalofChongqing MedicalUniversityTheincidenceatesofrhythmiaandtransientS-changeserecomparedat24and72ous，swelasthe ratedifferencebetweenthedetectionrateofarrhythmiaandtransientST-Tchangesat72hoursandthedetectionrateat24hoursin thecontextofvaiousriskfactorssuchasage，hypertesion，yperlipidemia，adcoronaryheartdisease.Results：mongthe6ptientswithpalpitation，thedetectionratesofvarious typesofarhythmiaandtransientST-Tchangesat72hoursweresignificantly higher than those at 24 hours（ P<0.05 ）.The rate difference of atrial premature beats in patients aged <60 years was significantly higher than that in patients aged ⩾ 60 years（ P <0.01）.The rate difference of atrial premature beats in hypertensive patients was lower than that in non-hypertensive patients（ P <0.05）.The rate diference of T-wave changes in patients without hypertension or coronary heart disease was greater than that in patients with hypertension or coronary heart disease （ P<0.01 ）.Compared with 24-hour three-lead ECG monitoring，72-hourtree-leadECGmonitoring withAPcansignfcantlyimprovethedetectionrateofarhythmiaandtransientST-T changes.In patients with palpitation aged <60 years or without hypertension，72-hour three-lead ECG monitoring with AP is more sensitivetoatrialprematrebeatshichareosistent withself-perceivedsymptoms，andinpatientswithpalpitationwithouthpeten sionorcoronaryheartdisease，72-hourthree-leadECGmonitoringwithAPismoresensitivetotransientT-wavechangesssociated withsymptoms.Conclusion：Thetechniqueof72-hourthre-leadECGmonitoring withAPgreatlypromotestheaccuracyandtimelinessofdiagnosis，reduces the economic burden of patientswith palpitations，and optimizes the allocationof medical resources. 【Key words】long-term electrocardiogram monitoring;palpitation; ambulatoryelectrocardiography;arrhythmia;ST-Tchanges
