Abstract：ObjectiveTo explorethe clinical characteristics，treatment strategies and related considerations of acuteinfarctionof artery of Percheron（AoP）with negative MRIfindings.MethodsAretrospective analysis was conducted on thediagnosis and treatment processof a case of acute infarction of the AoPwith a negative MRI combined withareviewofrelevantliterature.ResultsThepatient initiallpresented withdizziness，which gradually worsened tocoma.During treatment，antiplatelet therapyand other measures to treat cerebral infarction were administered.The initial MRI DWI results were negative，and emergency angiographyand postoperative CTA did not revealany large vessel occlusion.Onthe third day post-surgery，a CTscan indicated bilateral thalamic infarction， consistentwith the diagnosis of acute infarction of the AoP.ConclusionsMRIDWI imaging inacute infarction of theAoPmayoccasionallyyield negativeresults，a phenomenon thatis exceptionallyrare.The clinical presentationof this disease is varied，and earlydiagnosis poses a significantchallnge.Afterruling out intracranial hemorhage，the possibilityof thisdisorder shouldbecontemplated，andearlyimplementationofantiplatelettherapyandotherrelevant measures are advocated.
