帕金森患者脑萎缩对于丘脑底核脑深部电刺激术中气颅形成以及术后电极位置偏移的影响

Abstract: Objective To investigate the influence of brain atrophy on pneumocephalus during subthalamic nucleus deep brain stimulation(STN-DBS)and postoperative lead displacement in Parkinson's disease(PD). MethodsA retrospective analysis was conducted on 88 Parkinson‘s patients with bilateral STN-DBS admitted to Beijing Temple of Heaven Hospital affiliated with Capital Medical University from October 2018 to June 2O21. The preoperative and one month postoperative clinical evaluation data of all patients were collected.The volumes of various brain regions were assessed by preoperative magnetic resonance imaging(MRI).The electrode location wasreconstructed based on preoperative MRI,immediate postoperative and longer follow-up computed tomography(CT). Unilateral pneumocephalus volume percent(uPVP) was also evaluated by using immediate postoperative CT. ResultsPreoperatively,PD patients with more severe brain atrophy exhibited poorer preoperative levodopa response( P=0.007 ).Intraoperatively,a significant negative correlation was observed between cerebral cortical volume and uPVP( P=0 .039). Postoperatively,a trend toward a negative correlation was observed between cerebral cortical volume and the distance of electrode displacement( P=0.069 ). ConclusionsBrain atrophy represents an important factor affecting both the pneumocephalus formation during STN-DBS and postoperative electrode displacement.
