【Abstract】 Objective To analyze the morphological changes of peripheral blood cells in elderly patients with chronic obstructive pulmonary disease (COPD)combined with pulmonary infection, and to provide reference for clinical diagnosisand treatment.MethodsFrom March1,2024 toOctober31,2024,82elderly patients with COPD who received treatment in Shanxi Provincial Hospital of Integrated Traditional Chineseand Western Medicine were selected as the study objects. The patients were divided into COPD group(41 cases)and COPD group (41 cases)according to whether they had pulmonary infection. After admision, pathogens, lung function, peripheral blodcell morphologyand infection-related inflammatory factors were detected insequenceinall patients.Basedon theanalysisofthedistributionand compositionof pathogenicbacteria, thecorrelation betweenpathogenic bacteria and infection-related inflammatory factors was studied bycomparing the morphological results of peripheral blood cells.ResultsA total of 53 strains of pathogenic bacteria were detected in41 COPD patients with pulmonary infection.Gram-negativebacteriaaccountedfor67.9%,folowed by Gram-positivebacteria 22.6%.Theabnormal rates of leukocyte and erythrocyte morphology in COPD grade II were higher than those in COPD grade I,and those in COPD grade II were higher than those in COPD grade I and II .The abnormal rates of leukocyte and erythrocyte morphology in COPD group were significantly higher than those in COPD group. There were significant diferencesinpulmonaryfunctionand levelsof infection-related inflammatory factors inpatientswithdiferent COPD grades. The abnormal morphologyof white bloodcells and red blood cells was negativelycorrelated withlung function indexesand positively correlated with infection-related inflammatory factors( P <0.05).Conclusion The abnormal changes of peripheral blood cell morphology in the elderly patients with COPDcomplicated by pulmonary infection are closely related to lung function and infection-related inflammatory factors.
