【Abstract】 Objective To explore the application value of hemodynamics based on real-time three - dimensional echocardiographycombined with bloodflowvector imaging in thediagnosis of children withcomplex congenital heart disease.MethodsAretrospective analysis of 7O children with complex congenital heart disease who received surgical treatment from January 2O21 to January 2O24 was performed.Real-timethree-dimensional echocardiography parameters and bloodflow vector imaging parameters were compared between the two groups.Multi -factorlogistic of hemodynamic indexes inchildren with complex congenital heart disease,hemodynamic indexes in diagnosing complex congenital heartdisease inchildren.ResultsThe rightatrial diameter(RAD),rightventricular end-diastolic diameter(RVEDD)and main pulmonary artery diameter (PAD)while the left ventricular end-diastolic diameter(LVEDD)wassignificantly lower than that incontrol group( P<0.05 ).Thediastolicpeakvelocity(Vp),peak flowrate (Fp)and total forward flow rate(DQ + )in the basal segment,middle segment and apex segment in observation group were significantly higher than those in control group ( P <0.05). Logistic regression results showed that RVEDD,PAD,Vp,Fp and DQ + were independent risk factors for children with complex congenital heart disease.ROC curve results showed that PAD and DQ + alonehada high valuein the diagnosis ofcomplex congenital heart disease inchildren (AUC>0.7),and thecombineddiagnostic eficiency ofRVEDD,PAD,Vp,FpandDQ + was significantlyimproved,withAUC=O.899,sensitivity70.O% and specificity 92.9% .ConclusionReal-time threedimensional echocardiography parameters RVEDD and PAD combined with blood flowvector imaging parameters Vp, Fp and DQ + can provide important reference value for screening children with complex congenital heart disease.
