Application of comprehensive management model in community chronic disease management of diabetic patients

【Abstract】Objective： To explore the effect of implementing comprehensive management model in community chronic disease management of diabetic patients. Methods： From February 2019 to February 2020， 76 patients with diabetes in the community were selected and grouped according to different management methods， with 38 cases in each group. The reference group was given the basic chronic disease patients， and the experimental group was given the comprehensive management mode.
