Exploration on the Effect of Lumbar Facet Joint and Multifidus Muscle Degeneration on Degenerative Lumbar Spondylolisthesis Based on the “Sinew-Bone Imbalance” Theory

Abstract："Sinew-bone balance"isacore factor inmaintaining lumbar stability，and its decline significantly contributes tothe pathogenesis of degenerative lumbarspondylolisthesis（DLS）.Collctively，"sinew"and "bone" form a dynamic-static equilibrium system in the lumbar spine.Within this system，lumbar facet joints and the multifidus muscle play indispensable roles in stabilizing the lumbar spine due to their uniqueanatomical positioning. Pathologically，these structures interact reciprocall：the multifidus muscle，asan important part of "sinew，its atrophy can break the local stress balance of the lumbar segment，thus leading to "sinew-bone imbalane." This condition further accelerates degenerationofboth facet joints andthemultifidus muscle，ultimately triggering DLS.This progression exemplifies the pathogenesis of "sinew-bone imbalance originating from sinewdysfunction."Therefore，this articleemploys the "sinew-boneimbalance"theoryasaframeworktoinvestigate the mechanismofactionoflumbarfacet jointand multifidusdegenerationin disrupting lumbardynamic-staticequilibrium，aiming toenrich thescientific foundation of the "sinew-bone" theory in traumatology of traditional Chinese medicine.
