Diferential diagnosis for recurrnce and treatment response of glioma based on multimodal MRI radiomics

[Abstract]Objective:Toinvestigatethediferentialdiagnosticvalueforrecurrenceandtreatmentresponseofgliomabasedon multimodalMRIradiomicsmodels.Methods:Aretrospectiveanalysiswasperformedfor110patientswithglioma,allof whom were examined by 3.O T MRI scans,including T1 FLAIR,TWI,TFLAIR,DWI,T,WI-CE and DCE-MRI. 11O cases had abnormalenhancementonpost-treatmentMRIimages.Accordingtothepathologicaldiagnosisand/ormultipleclinicaland imagingfollw-upexaminations,11Ocasesweredividedintotherecurrncegroup(77cases)andpost-treatmentresponse group(33cases).And11Ocasesweredividedintothetrainingcohortandvalidationcorhortataratioof7:3.Theimages wereimportedintothe3DSliceropen-sourcesoftware,andthelesionsweremanuallydelineatedandtheVOIs were reconstructed,from which the radiomics features were extracted.The -test,MUSE selector and LASSO algorithm were used toscreenfeatures.The RandomForest(RF)wasusedtoconstruct thediagnosticmodel,andtheROCcurve wasused toevaluatethediagnosticperformanceofthemodel.Results:TheTWI,ADC,TW-CE,Ks,V,iAUC,DCE-MRI+TW-CE modelshad7,5,3,7,5,10,5features,andachievedtheAUCsinthevalidationcohortof0.716,0.759,0.836,0.741, 0.836,0.897,.922.Conclusions:ThemultimodelMRIradiomicsmodelshavegooddiagnosticeficacyindiferentiatingglioma recurrenceandpost-treatmentresponse.Comparedwith thesinglesequence model,thecombined modelhas higheracuracy, specificityand sensitivity.

脑胶质瘤是中枢神经系统原发肿瘤中最常见的神经上皮肿瘤,占脑肿瘤的 81%[1] ,经标准疗法治疗的患者预后差,复发率高,高级别脑胶质瘤复发率可达 90%[2-3] 。(剩余7752字)
