Trajectory of fatigue and related influencing factors in patients with inflammatory bowel disease

[ABSTRACT]ObjetiveToinvestigatethetrajectoryoffatigueandrelatedinfluencingfactors inpatients with inflammato ryboweldisease（IBD）．MethodsAtotalof290IBDpatients whoattendedDepartmentofGastroenterology，TheAfiliated Hospitalof Qingdao UniversityfromJuly2023 toMay 2024 wereenroledassubjects usingthemethodofconvenience sampling. Demographicandclinicaldata wererecoded，andtheseverityoffatigue wasasessedatbaselineandat3and6monthsafteratendingthe hospital.Thelatent profileanalysis （LPA）wasused toidentifythepotentialsubgroupsoffatigue symptom trajectories， andthenthepatientsweredividedintodiferent groups.Aunivariateanalysiswasusedtoidentifytheindicators withstatisticalsig nificance indemographicandclinicaldataacross groups，andtheabove indicatorswere included inthe multivariatelogisticregres sion model todeterminetheinfluencing factors fordiferentfatigue symptom trajectories.ResultsTheLPAmodelidentified fourdistinctfatiguetrajectoriesamongIBDpatients，ie.lowfatigue，moderatefatiguewithremision，highfatiguewithdecline， andhigh fatiguewithpersistence.Theunivariateandmultivariatelogisticregresionanalysesshowedthatage，sex，bodymasindex，presenceofanemia，medication（hormoneorbiologicalagent），disease activity，sleepquality，andthe statusofanxietyand depression were influencing factors for fatigue symptom trajectory（ ⌈P<0.05⌉ .ConclusionThere are four distinct fatigue trajectories，lowfatigue，moderatefatiguewithremision，highfatigue withdecline，andhighfatigue withpersistence，inIDpatients withinashortterafterinitialdiagnosis.Themedicalstaffcanearlyidentifythepatientswithhighfatigueandimplementtargeted andindividualizednursinginterventionsbasedontheinfluencingfactorsforfatiguesymptom trajectories，therebyimprovingthe outcome of fatigue in IBD patients.
