【Abstract】 Objective To investigate the value of ultrasound nuchal translucency thickness (NT)and ductus venosus (DV) blood flow parameters in early pregnancy on guiding fetal heart structure judgment.MethodsA retrospective analysis wasperformed on1O2 pregnant women who received ultrasound examination in early pregnancy from March 2022 to January 2025 in Song Coutry People's Hospital.Among them,1 case was confirmed to havefetalcardiacstructuralabnormalitiesbyfetalsurgery,3casesbyautopsy,7cases byfolow-up within1week to 3 months after delivery,and 91 cases of normal heart structure were confirmed.The types,composition ratio, ultrasound detection rate and ultrasonic manifestations offetal heart structure abnormalities were compared.The differences inNTandDVparameters [preload index (PLI),venous pulsatilityindex (PIV),peak ventricularsystolic velocity/maximum atrial systolic reflux velocity(S/A)] were compared.Receiver operating characteristic (ROC) curve wasdrawn to evaluate the diagnostic eficiency of NTand DV parameters on judging fetal heart structure abnormalities.ResultsAmong the types offetal heart structureabnormalities,ventricular septal defect accounted for36.4%(4/11).Comparedwithfetuses withnormalheartstructure,NT,PLI,PIVandS/Ainfetuseswithabnormal heart structure were higher( P <0.001).ROCcurveshowed that thesensitivities ofNT,PLI,PIVand S/A inthe diagnosis of fetal heart structure abnormalities were 64.84% , 95.60% , 76.92% and 69.23% ,and the specificities were 90.91% , 45.45% 90.91% and 90.91% ,and the corresponding cut-off values of Youden index were 2.54 mm, 0.54,0.58and 2.46.ConclusionUltrasound detection of NTand DV blood flow parameters in early pregnancy hasgooddiagnosticvalueon fetalheartstructureabnormalities,andcanbeusedasanon-invasiveandconvenient early screening tool.

胎儿心脏结构异常是一种较为常见且严重的出生缺陷,包括室间隔缺损、房间隔缺损、法洛四联综合征、主动脉缩窄等,这不仅会阻碍胎儿宫内生长发育、降低其出生后生存质量,还会因长期诊疗需求给家庭带来沉重的经济压力,同时引发孕妇及家属焦虑、担忧等心理负担。(剩余5956字)
