Evaluation of the effect of high artificial rupture ofmembranes following failed low-dose oxytocin induction

Abstract:Objective:Toevaluate theeficacyof highartificial ruptureof membranesandits impactonmaternal and neonatal outcomesafter failureof induction with a water baloon combined with low-dose oxytocin.Methods :From January 2020to December 2O23,atotalof 5Opregnant women who had indications forlabor inductionand experienced failedinductionafter2-3daysofusingacervicalripeningbalooncombined withlow-doseoxytocininourhospital were selected.They were divided intoa control group ( n=24 )and an observation group( n =26).The control group continued oxytocininfusionorunderwentcesareansection,whiletheobservationgroupreceived highAROM folowed byoxytocin infusion after one hour.Cervical Bishop score,total labor duration,cesareansection rate,postpartum hemorhage, postpartum infection rate,neonatal Apgar score,and intrapartum complications (e.g.,placental abruption,fetal distress)were compared.Results:The cesarean section rate was significantly lower in the observation group (30.77% ) than in the control group( 100.00% ), P<0. 05 . Compared with the control group,the observation group showed a less postpartum hemorrhage volume ( P<0. 05 ),a shorterof the totallabor duration ( P<0. 05 ),and a higher neonatal Apgar score ( P<0. 05, .There wereno significant diferences inamnioticfluid characteristics,postpartum infectionrate,orintrapartum complications between the two groups (P>0.05 ).Conclusion:Incases offailed induction with awaterballoon and low-dose oxytocin and a cervical Bishop score <6 ,high artificial rupture of membranes significantly promotes labor progress,increases therateofvaginaldelivery,reducesthecesareansectionrate,anddemonstratesafavorablesafetyprofileforbothmother and neonate,with positive clinical significance.
