Abstract： ObjectiveTo explore the clinical characteristics electric bicycle-related traumatic brain injury（TBI）in children，and to provide novel insights for the prevention and treatment pediatric TBI. MethodsThe clinical data 267pediatric patients with electric bicycle-related TBI admitted to the ， Hospital， from January 1，2O22，to June 30，2O24，were retrospectively analyzed，and the risk factors were identified using a Logistic regression model. ResultsA total 267 children were enrolled in this study. Among them，92 cases （34.46% ）sustained injurieswhile riding electric bicycles，and 175 cases（ 65.54% ）were hit by electric bicycles.Specifically，19 children（7.12%）experienced one episode vomiting，35 cases （13.11% ）had two or more vomiting episodes，and 53 children （ 19.85% ）presented with disturbance consciousness.Intracranial abnormalities were detected in 49 cases，with an incidence rate 18.35% .Additionally，9 patients showed negative findings on cranial computed tomography but positive results on magnetic resonance imaging. Logistic regression analysis indicated that two or more vomiting episodes and disturbance consciousness were risk factors for intracranial abnormalities in children with TBI. Conclusions Disturbance consciousness and two or more times vomiting are core early warning indicators for intracranial abnormalities. MRI has significant supplementary diagnostic value for occult injuries that are negative on CT. These findings can provide references for optimizing clinical diagnosis and treatment processes，reducing radiation exposure，and improving prognosis.
