超声引导中线指引联合局部变焦桡动脉穿刺置管方法在胸腔镜手术患者麻醉中的应用

【Abstract】ObjectiveToinvestigate theeficacyof midline guidancecombined with local zoominultrasound-guided radialarterypunctureandcatheterization inpatientsundergoing thoracoscopic surgery.MethodsAtotalof8Opatients who underwent selective thoracoscopic surgery from March 2023 to March 2O25 wererandomly divided into experimental group （group R ）and control group（group C），with 40 patients in each group.Both groups underwent ultrasound-guided radial arterypuncture andcatheterization：thegroupCwere given conventional ultrasound guidance，whilethe group R were given midlineguidancecombinedwithlocalzoom.Andthen，first-attmptsuccessrate，total successrate，puncturetim，localizationtime，andtheincidenceof complications （arterial spasm，local hematoma，and local bleeding）between the twogroups were compared. ResultsThe first-attempt success rate in the group R（ 87.5% ）wassignificantly higher than that in the group C （ 67.5% ） χ2=4.588 ， P=0.032 ）；the puncture time （ {14.12±2.53 ）s and localization time（12.08±2.14）s were significantly shorter than those in the group C（ 18.96±3.05 ）sand（13.41±2.58）s（ t -valueswere 7.725 and 2.509，respectively，all P<0. 05 ）.The incidence of arterial spasm （ 2.5% vS 25. 0% ），local hematoma（ 5. 0% Vs 25.0% ），and local bleeding（ 2.5% vs 22.5% ）was significantly lower than those in the group C（ χ2 values were 8.538， 6.275 and7.314，respectively，all P<0.05 ）. There was no statistically significant difference in total puncture success rates between the two groups （ 95.0% vs 87.5% ， χ2=0.626 ， P=0.429 ）.Conclusion Midline guidance combined with local zoomtechnologyunder ultrasoundguidancecanimprovethefirst-atemptsuccess rateofradialarterypuncture in patients undergoingthoracoscopic surgery，shorten theoperation time，and reduce the incidence of complications，which ismore advantageous than the traditional ultrasound-guided puncture method.
