【Abstract】ObjectiveTo evaluate theeficacyandsafetyof remimazolamcombined with propofol-etomidate forpainles gastrointestinal endoscopy inelderly patients.MethodsA total of 244 elderlypatients undergoing painless gastroenteroscopy were selected as research subjects and randomly divided into remimazolam besylate + propofol-etomidate mixture group （group RA， n=122 ）and propofol-etomidate mixture group （group C， n=122 ）.The systolic blood pressure（SBP），diastolic blood pressure（DBP）and heartrate（HR）ofthetwo groups werecomparedat diferent timepoints：before anesthesia induction（T1），when the gastroscope passedthrough the larynx（T2），atthe startofcolonoscopy（T3），when thecolonoscope passedthrough the hepatic flexure（T4），andatthetime of awakening（T5）．Besides，the onsettime，time to leave theroom，total dosagesof propofol-etomidate，andtheincidences ofadversereactions includingdecreasedoxygensaturation （ Sp02 ），myoclonus，hiccups，coughing，body movement，somnolence，injection pain，dizziness，nausea and vomiting，and delirium were also compared between the two groups.ResultsThedecrease in SBPandHRin thegroupRA wassmallr thanthatinthegroupC，theonsetof anesthesiaanddurationofaction werelongerthanthoseinthegroupC，thetotal dose of propofol-etomidatewaslessthanthatinthegroupC，andtheincidenceofmuscle tremorsandinjectionpainwaslowerthan that in the group C，and differences were statistically significant （ P<0.05 ）.There was no statistically significant difference in the incidence of other adverse reactions such as decreased SpC -2 ，hiccup，coughing，body movement，and somnolence between the two groups （ P>0.05 ）.ConclusionBased on the analysis of anesthesia effects for painless gastrointestinal endoscopy intheelderly patients，propofol-etomidate mixture combined with appropriateamountof remimazolam can prolong theonsettimeand exit time，butitshowscertainadvantages inmaintaining patienthemodynamicstability，reducing muscle tremors and the incidence of injection pain.
