2743例儿童患者分泌物培养特点及其治疗策略分析

【Abstract】ObjectiveToanalyzethedistributionanddrugresistancecharacteristicspathogenic bacteria insecretion culturespecimensfromchildrenwith infections inahospital in ,soas toprovidereference strategiesforclinical empiricaltreatment.MethodsAretrospectivecollction 2743 pediatricsecretionculture specimens from2019 to2023 wasconducted inthe hospital.WHONET5.6stwareand CLSI standardswereused toanalyzethedistribution pathogensand drug sensitivity results.ResultsThe positive detection rate specimens was 19.76% (542/2743),witha total 561 strains pathogenic bacteriaisolated.Among them,there were 428 strains Gram-positive bacteria( G+ bacteria) ( 76.29% ),114 strains Gram-negative bacteria ( G- bacteria)( 20.32% ),and 19 strains fungi (3.39% ). The top five pathogenic bacteria were Staphylococcus aureus (55.08 % ),Staphylococcus epidermidis(9.80 % ),Escherichiacoli ( 4.46% ),Klebsiellapneumoniae( 3.39% ),and Pseudomonas aeruginosa (2.85 % ).The drug sensitivity results showed that the detection rate methicillin-resistant Staphylococcus aureus(MRSA)was 31.1% ;the resistance rates Escherichia coliandKlebsiellapneumoniaetomostantimicrobialdrugswerelowerthanthenationalmonitoringlevel(exceptforlevloxacin).Among the positive cases,tonsillar/adenoidal hypertrophy accounted for 34.50% ,and neonatal omphalitis,neonatal infection,etc.accounted for 24.54% .Such patients can be empirically treated with β -lactamase inhibitors or first-and second-generationcephalosporins.ConclusionThepathogenicbacteriaculturedfromchildren'ssecretions inthis hospitalare mainly G+ bacteria.In clinical practice,antibacterialdrugs should beselected acording to the distributionand drugresistance characteristics pathogenic bacteria to improve the eficacy and reduce theoccurrence drug resistance.
