Identificationof Mycoplasma hominis Bacteremia byMALDI-TOFMS Combined with 16S rRNA Gene Sequencing

Abstract: ObjectiveTo analyze the blood culture samples of a patient with postpartum fever by Matrixasisted laser desorption/ionization-time of flight mass spectrometry (MALDI-TOF MS) and 16S rRNA gene sequencing,inorder to identifythe cause of postpartum fever and provide a reference for clinical diagnosis and treatment.MethodsThe positive cultures in the blood culture bottle were transferred to columbia blood AGAR plate and chocolate AGAR plate, incubated in 35∘C , 5% (2号 CO2 incubator, and examined by direct gram staining. The target strainson the medium were stainedand identified by MALDI-TOFMS.The target strains were examined for antimicrobial susceptibility based on the results of MS identification,and their identity was confirmed using 16S rRNA gene sequencing techniques.ResultsThe positive culture in the blood culture bottle was detected by Gram staining microscopy,but no bacterial structure was found.Colombia bloodAGAR plateculture for 48 hours can be sen in needle-like transparent smallcolonies,with a chocolate AGAR plate with no bacterial growth. Mycoplasma hominis was identified by MS on columbia blood AGAR with 99.9% confidence. It was resistant to other medications andsensitive tominocycline,doxycycline,lucamycin,and clindamycin,according tothe resultsof the antimicrobial susceptibility test. The nucleotide homology of the 16S rRNA gene sequence of the target strain was 99.71% with that of the standard strain of Mycoplasma hominis ATCC23114. Conclusion The cause of postpartum fever is
Mycoplasma humanemia.The results of MALDI-TOF MS were reliable for the identificationofMycoplasma hominis. 16S rRNA gene sequencing combined with MALDI-TOF MS can improve the diagnostic eficiency and accuracy of rare pathogens.
