Abstract: Objective To explore correlation between peritumoral edema volume(PTEV) and progression-free survival(PFS) as well as overall survival( OS) in IDH wild-type glioblastoma (GBM) patients, providing a reference for clinical surgical planning and prognostic assessment. MethodsA total 98 IDH wild-type GBM patients who underwent surgical treatment in , from January 2O2O to December 2022 with complete follow-up data were included. Clinical data,postoperative chemorapy and radiorapy information,PFS,OS,pathological reports,and imaging data were collected. Imaging data were processed using 3D-Slicer to calculate tumor volume parameters,and X-tile stware was used to determine optimal cutf value for PTEV based on minimal P-value method,dividing patients into PTEV <50cm3 and PTEV⩾50c m groups. Univariate and multivariate Cox regression analyses were performed to explore prognostic factors.ResultsThe median follow-up time for 98 patients was 26.8 months.By end study,9O patients had died,and 8 patients were still alive.Univariate analysis revealed that Ki-67 positivity rate ⩽30% , PTEV<50cm3 ,edema-tumor ratio(ETR) <1.5 ,and extent resection(EOR) 98% were associated with prolonged PFS and OS.Multivariate analysis showed that Ki-67 positivity rate,ETR,and EOR were independent factors affcting OS and PFS,while PTEV was an independent factor for PFS,but its independence regarding OS was not clear. ConclusionsIn IDH wild-type GBM patients, those with PTEV <50 cm³ have longer PFS and OS than those with PTEV⩾50 cm³.The extent tumor resection and Ki-67positivity rate are important prognostic indicators.It is recommended to maximize safe resection peritumoral edema while preserving neurological function and follow this with regular postoperative radiorapy and chemorapy to improve patient survival.
