Abstract： Objective To investigate the surgical strategy one-stage total resection tumors in the thoracic spinal foraminal area，and to establish a new surgical classfication tumors in this area.Methods clinical data 41 patients with tumors in thoracic intervertebral foramen region admitted to the ，the Afliated University from January 2O18 to December 2O24 were analyzed retrospectively. Forty-one tumors were treated with surgery according to the new clasification. According to the tumor location within the thoracic spinal canal，inside or outside the foramen，as well as the relationship between the tumor and the dura，the tumors in the intervertebral foramen area the thoracic spinal canal were divided into three types： type I was the tumor occupying the spinal canal and the foramen，type I 2 was the tumor spanning in the three regions the spinal canal，the foramen and outside the vertebral foramen，and type I wasthe tumor within and outside the intervertebral foramen. According to the relationship between the tumor and the spinal dura，type I is divided into type I，the tumor is epidural， type ，and the tumor communicates with the dura. ResultsThirtynine cases achieved one-stage total resection，with a total resection rate 95.1% . symptoms all 40 patients were significantly improved after surgery，with an improvement rate 97.6% ，and there was no significant change symptoms in 1 patient. After operation，2 patients developed fever and 2 cases developed interscalene thrombosis the lower extremities. Conclusions improved typing system can accurately guide the selection surgical approach，and combine microscopic technology and stability assessment to achieve a balance between total tumor resection and spinal functional protection.
