Predictive value of heart rate deceleration capacity and QT dispersion for major adverse cardiovascular events in patients with acute myocardial infarctionZHAO Yican,DU Xinchao, ZHANG Yameng (Department of Cardiopulmonary Function, Zhengzhou Central Hospital, Zhengzhou Henan 45O0o,China) [Abstract]ObjectiveTo analyze the predictive value of heart rate deceleration capacity(DC)and QT dispersion(QTd) for major adverse cardiovascular events (MACEs)in patients with acute myocardial infarction (AMI).MethodsA retrospective analysis was conducted on 8O AMI patients who had allundergone percutaneous coronary intervention (PCI),designated as an observation group.Based on theoccurrence of MACEs during folowup,they were further divided into an occurrence group(2O cases) and a non-occurrence group(6O cases). Additionaly,80 healthy examinees during the same period were selected as a control group. Heart rate DC and QTd were compared between the observation group and the control group,as well as between the occurrence group and the non-occurence group from the observation group.The predictive value of heart rate DC and QTd for MACEs in AMI patients was analyzed by using ROC curve analysis.ResultsAmong the 8O AMI patients in the observation group,2O experienced MACEs during follow-up, accounting for 25.00% ( 20/80) . The heart rate DC in the observation group was significantly lower thanthat in the control group,whiletheQTd wassignificantly higher (both P<0.01 ). The heart rate DC in the occurrence group was lower than that in the non-occurrence group,and the QTd was higher(both P<0.01 ). ROC curve analysis showed that the combined assessment of heart rate DC and QTd had a higher predictive value for MACEs in AMI patients ( AUC=0.958 ) compared to either parameter alone ( AUC=0.895 and O.812,respectively). ConclusionHeart rate DC and QTd could predict the risk of MACEs in AMI patients,and their combined use offers higher predictive value.
