Abstract:Objective Toassess theasociationbetweenurinarylevelsofvolatileorganiccompoundmetabolites (mVOCs)and risksofmetabolicdysfunction-asociatedsteatoticliverdisease(MASLD)inthegeneraladultpopulation.MethodsBasedon4 cyclesofcross-sectional surveysfromthe201-018National HealthandNutritionExamination Survey (NHANES), generalizedlinearmodelswereemployedtoevaluatetheassociationsbetweenindividualmVOCexposuresandtheriskof MASLD.A two-stage Least Absolute Shrinkage and Selection Operator-Weighted Quantile Sum (LASSO-WQS)regression model wasconstructed toinvestigatetherelationshipbetweenmixed mVOCs exposuresand MASLDrisk,andtherelative contributionsoftheindividualcompoundswerequantitativelyanalyzed.ResultsThesingle-exposureanalysisrevealed significantpositiveasoiationsof-aminothiazoline-4carboxylicacid(CA),N-acety-S-2-arboxyethyl)-cyteine (CEMA),and N-acetyl-5-(3,4-dihydroxybutyl)-L-cysteine (DHBMA)with MASLDrisk after adjusting forconfounders.Inthe two-stage mixed-exposure analysis,the first-stage LASSOregressionidentified6mVOCs with stronger association with MASLD risk.The second-stage WQS regression demonstrated a statisticallysignificant positive asociation betweenmixed mVOCs exposures and MASLD risk (OR=1.306, 95% CI: 1.132-1.507; P<0.001 ),withCEMA contributing the highest weight (36%) .ConclusionThestudyrevealsasignificantpositiveassociationbetweenurinary levelsofmVOCsmixturesand MASLDrisk,suggesting potential hepatotoxic efects ofVOC (especially CEMA)exposures, whichurges future mechanistic studies of VOC mixture-related health impactsand listing of CEMA for health risk control.
