降脂祛斑方多成分协同调控炎症-代谢网络改善2型糖尿病合并高脂血症:网络药理学与临床验证

Abstract:ObjectiveToexplorethe therapeutic mechanismofJiangzhi Quban Recipe (JZQBR)for type2diabetes melitus (T2DM)complicated withhyperlipidemia and validateitsclinical eficacyand safety.Methods Theactivecomponentsand disease targetsof JZQBR were screeed using TCMSPandGeneCards databases,followed byprotein-protein interaction analysis and GO and KEGG enrichment analyses. In the animal experiments, ApoE-I- mice were randomized into blank control, model,simvastatintreatmentndlow-andigdoseJQBRgroups.Inelatter4groups,themicewerefedaighatdiet for24weekswithcorrespondingtreatmentsfromWeeks9to24.Thechanges inbodyweight,blood glucose,lipids,liver pathologyandifmatoryoiepiosofeiceeeaed.Iicalsdy7ptsith hyperlipidemia wererandomized equallintocontrol group for treatmentwithmetforminplusempagliflozin and JZQBR group withadditional JZQBRfor12consecutive weeks.Results Network pharmacologyidentified 65potential targets,with quercetineoldolieotdI-6,I-1β, α asthekeytargets.Thetargetswere enrichedmainly in the pathways involving inflammatory responses and diabetic complications. In the ApoE-1 mouse models, JZQBR treatment dose-dependentlyimproved bodyweight,blood glucose,and blood lipid profiles,andhigh-dose JZQBR producedastrongerefectthansimvastatinforimproving hepaticsteatosisandsignificantlyreduced inflammatorycytokine levels.Intheclinical tal,29patients inJZQBRgroupand31inthecontrol groupcompletedthetrial.The patientsinJZQBR groupshowedsignificantimprovementsinbodyweight,FG,G,Hblc,andliverenymeswithignificantlylowersing blood glucoselevel thanthecontrol group.The total effectiverates were comparable betweenthetwo groups.Conclusion JZQBRimproves T2DMcomplicated withhyperlipidemiaposiblybymult-argetregulationof theinflammation-metabolism network.
