Clinical study laparoscopic high ligation hernia sac combined with IPOM and local anessia Lichtenstein repair in treatment male patients with inguinal hernia after radical cystectomy

Abstract： ObjectiveTo compare clinical effcacy laparoscopic high ligation hernia saccombined with intraperitonealonlaymesh （IPOM）andlocalanessiaLichtensteinrepairfor treatmentinguinal herniaafter total cystectomy inmale patients.MethodsAtotal 59 male patients whounderwent totalcystectomy were included in this studyand randomly divided into two groups： laparoscopic high ligation hernia saccombined with IPOM group （IPOM group，30cases）andlocalanessiaLichtensteinrepairgroup（localanessia group，29cases）.The patientage， surgical time，postoperative ambulation time，visual analogue scalescorefor painon1stto 3rddaysand3months after operation，timetirstmobilzationpostoperation，paiscoresonpostoperativeday1，2and3，painscoresat3monthspost operatio wereobservedandcompared between two groups.Dataintraoperativebloodlos，hospital stay，timeto return todailyactivities，total surgerycosts，recurrenceratesatlyearafteroperationandcomplicationswerealsoobservedand compared in two groups patients.The results were analyzed to determineoptimal surgical method.ResultsThe IPOM groupdemonstratedbeteroutcomes intermssurgicaltime，time tistmobilization，painscores onpostoperativedays1， 2and3，painscoresat3months，intraoperativebloodlos，hospital stayandtime toreturn todailyactivitiescomparedto those local anessia group （ P<0.01 ）.However， total cost operation was higher in IPOM group than those local anessia group （P<0.01 ）.There were no significant differences inpostoperative complicationsbetween two groups （ P>0.05 ）. The recurrence rate IPOM group one year after surgery was better than that local anessia group （P<0.05） .ConclusionLaparoscopic IPOM with high ligation hernia sac issuperior to traditionalLichtensteinrepair inreducing postoperativepainandrecurrencerates.Thissurgical methodshouldbe first choice when economic conditions permit.
