Pharmaceutical care for a ulcerative colitisand ankylosing spondylitispatient who developed pustulai psoriasisinducedby infliximab

ABSTRACTOBJECTIVEToprovideareference for pharmaceuticalcare inpatients withulcerativecolitis（UC）andankylosing spondylitis （AS）whodeveloped pustularpsoriasis induced byinfliximab.METHODs Clinical pharmacistspartcipated inthe pharmaceuticalcareprocessofapatientwithUCandASwhodevelopedpustularpsoriasisafterusing infliximab.Theclinical pharmacistsdetermined，usingNaranjo'sScale，thatthecorelationbetweenthepatient’spustularpsoriasisandinfliximabwas likely".Regarding thepatient’sdevelopmentof pustular psoriasisafterusing infliximab，theclinical pharmacistsrecommended discontinuinginfliximabandswitchingtoUpadacitinibextended-releasetablets.Forthepatient’sskinalergicreactionafterusing upadacitinib，theclinicalpharmacistsadvisedcontinuingtheuseofupadacitinibandcloselymonitoringanypotentialadverse reactionsduringthetreatmentperiod.RESULTSThecliniciansadoptedthecliicalpharmacists’recommendation.Followingthe treatment，thepatient’ssymptomsweresignificantlyaleviated，andthepatientwasdischargedwithmedication.Thefollw-up afterdischargeshowedthatthetreatment wasefectiveandwel-tolerated.CONCLUSIONSTeclinicalparmacistsanalyzedthe causalrelationshipbetween infliximaband pustularpsoriasis.Through pharmaceuticalcaremeasuressuchasdynamic monitoringof skinlesions，evaluationoftreatmentresponses，andoptimizationofdrugregimens，theyasistedthephysicians informulatingan

individualized medication plan， ensuringthe safety and efficacyof the patient’smedicationuse.
