Research Advances in Traditional Chinese Medicine Regulating Exosome Pathways for Ischemic Stroke Treatment

Abstract：Ischemic stroke（IS），a life-threatening condition caused by cerebrovascular occlusion，remains aleading causeof mortality.Exosomes，extracellular vesicles secreted bycells，hold significant potential for ISdiagnosisand therapy.Accumulating evidence demonstrates that active components of Chinese herbal medicine（CHM）and CHM formulascanamelioratepost-IS brain injurybymodulating exosomal pathways—mitigating blood-brain barrier disruption，attenuating neuroinflammation，inhibiting neuronal apoptosis，and promoting neurogenesisand angiogenesis.This review systematically summarizes the diagnosticand therapeutic roles of exosomes in IS，with particular emphasison the mechanistic advances in CHM-mediated exosomal regulation for IS treatment，thereby providing novel perspectives for CHM-based IS interventions
