【Abstract】ObjectiveTo evaluate the diagnostic value of ultrasound-guided fine needle aspiration biopsy (USFNAB)in papilary thyroid carcinoma (PTC) with nodules of diffrent maximum diameters.MethodsSixty patients with confirmed PTC through surgeryorpathological biopsywho visitedour hospitalfrom March 2O22 to August 2023 were selected as the PTC group,and 65 patients with benign thyroid nodules duringthe same period were selected as the benign nodule group.The cellular typing results were compared between two groups by surgery or pathological biopsyas the gold standard.The results of PTC detection were comparedbetween US-FNABand surgical or pathological biopsy,andtheconsistencyof US-FNABandsurgical or pathological biopsydetectionresults forPTC nodulesof diffrent maximum diameters was analyzed.ResultsThe proportion of type I,II,Iland IV patients in the PTC group was lower than that in the benign nodule group,while the proportion of type v and IV patients was higher than that in the benign nodule group( P< 0.05). Surgery or pathological biopsy as the gold standard,the detection rate of malignant nodules by US-FNAB was 96.8% ,the sensitivity was 96.8% ,and the missed diagnosis rate was 3.2% . A total of 62 nodules were detected in 6OPTC patients,the US-FNAB diagnostic results showed that 21 nodules were ⩽5 (20 mm,29 nodules were >5~10 mm,and 12 nodules were>10 mm.Surgical or pathological biopsy results: 22 nodules ⩽5 mm,30 nodules >5~10 mm,and10 nodules>10 mm.The consistency Kappa value of US-FNAB and surgical or pathological biopsy results for PTC nodules of diferent maximum-diameters was 0.844(P<0.001).ConclusionUSFNAB detection can be used to diagnose PTC nodules of different maximum-diameters,and theconsistency of USFNAB diagnosis resultsand surgical or pathological biopsy results for PTC nodules of diferent maximum-diameters is high.The US-FNAB methodcan be used for preoperative diagnosis of PTC nodules with smaller maximum-diameters in clinical practice in order to develop targeted treatment plans early and improve prognosis.
