【Abstract】 ObjectiveTo analyze the application value of magnetic resonance imaging (MRI) difusion tensor imaging(DTI)in the early diagnosis of recurrent neuromyelitis optica(RNMO).MethodsEighty patients with RNMO who visited the Second People's Hospital of Zhengzhou from December 2021 to December 2023 were selected as the RNMO group.In addition,8O patients with newly diagnosed neuromyelitis optica (NMO)who visited the same hospitalduring the same period were selected as the newly diagnosed NMO group,and 8O healthy individuals who underwent physical examinations atthe same hospital's examination center were selected as the healthy group.MRIDTI tests were performedon allthree groups,and the MRI-DTI parameters [partial anisotropy (FA)values and average diffsioncoeffcient (ADC)values] were compared.According to the extended disability statusscale (EDSS),RNMO patients were divided into three groups.The MRI DTI parameters of the mild group (2O cases,EDSS score ⩽3.5 points),moderate group (47 cases,4 points ⩽ EDSS ⩽6.5 points),and severe group (l3 cases,EDSS ⩾7 points) were compared.The receiver operating curve(ROC)was ploted to analyze the diagnostic eficacy of the MRI DTI parameters for RNMO.ResultsThe ADC value of the RNMO group was higher than that of the initial NMO group and the healthy group (P-3mm2/s and the optimal cutoff value for FA value was 0.38,the area under the curve (AUC)(95%Cl) for the combined detection of ADC value and FA value for diagnosing RNMO was 0.910(0.858,0.959).The sensitivity of the combined detection of ADC valueandFA value for diagnosing RNMO ( 96% )washigher than that of a single detection ( 79% , 72% 川 P <0.05). The specificity of the combined detection of ADC value and FA value for diagnosing RNMO(82 % ))was not significantly different from that of a single detection 80% , 84% ) (P>0.05). Conclusion The combined detection of MRI-DTI parameters ADC value and FA value has high sensitivity in diagnosing RNMO and can provide a reference for early clinical treatment.
