【Abstract】ObjectiveTo explore the diagnostic value MRI multimodal new sequence in preoperative T stage in patients with esophageal cancer.MethodsIn aretrospectiveanalysis，124 patients with esophageal cancer from December 2O20 to December 2O23 were divided into groups according to whether the lamina propria was broken， divided into the breakthrough group（T3-T4）and no breakthrough group（Tt-T2），allwith routine MRI sequence examination，IVIM-DWI examination，and DCE-MRI examination.3.0 T multiparameter MRI HR-TWI was used to evaluate tumor morphological characteristicsand functional nuclear magnetic parameters，general data，IVIM parameters and DCE parameters the breakthrough groupand no breakthrough group werecompared，and the diagnostic effcacy each parameter was calculated.Results Gender，age，BMI value，smoking history，and consumption frequency pickled between the breakthrough and no breakthrough group （ P> 0.05）. D and ADC [（0.70±0.15）x10-3 mm²/s， （1.07 ± （20 0.16） ×10-3 mm2/s] in the no breakthrough group were higher than the breakthrough group [（0.70±0.15）x10-3 mm²/s， （0.94±0.13） × 10 -3 mm²/s]，with statistically significant differences （ P <0.05）.The Ktrans [（O.31±0.07） min] in the no breakthrough group was significantly higherthan that in the breakthrough group[（O.24±O.O5）min]，with a statistically significant difference（P<0.05）.Kappa value =0.72forHR-TWI diagnosis，the Kappa value for IVIM-D diagnosis ≡ （20 0.69，and the Kappa value for DCE-Ktrans diagnosis =0.49 .The area under the curve for the combined HR-T2WI + IVIM-D+DCE-Ktrans diagnostic T stage was 0.912， sensitivity 100% ，specificity 96.1% ，accuracy 98.4%， positive predictive value 97.3% ，and negative predictive value 100% . Conclusion The combination HR-TWI， IVIM-D and DCE-Ktrans diagnosis significantlyand accuratelydetected the preoperative Tstage esophageal cancer and identifiedthelesion breaking through the muscularis propria，with high diagnostic efficacyand remarkableapplication effect.
