Establishment of a standardized management model for postoperative anti-osteoporosis medication in patientswith brittle fractures

ABSTRACTOBJECTIVETo investigate the establishment and promotionofanew standardized management model forantiosteoporosismedicationafterfragilityfracturesurgerybyresidentclinicalpharmacists，andprovidereferencesforresident pharmacists tocarryoutclinicalpharmaceuticalservices.METHODSFromJuly2023 toMarch 2024，595post-britefracture surgerypatientswereenrolled.UsingthePDCA（plan-do-check-act）cycle，residentcliicalpharmacistsidentifiedissuesand conductedinvestigationsinclinicalpractice.Throughintegratingclinicalpharmacistinterventionservicesbfore，duringandafer treatment，amedicationtreatmentpathwaywasdeveloped，therebyestablishingastandardizedmanagementmodelforantiosteoporosistreatmentfollowingfragilityfracturesurgery.LeveragingtheNationalBriteFractureBigDataPlatform（underthe National ClinicalResearch Center forOrthopedicsand SportsRehabilitation），adedicateddatamodulewasconstructed，providing bigdatasupport toevaluate theeficacyof thispharmaceuticalcare model.RESULTS Continuous PDCA cycle driven improvements significantly increased the proportion of osteoporosis diagnosis（from 9% before intervention to 81% ）and proportion of drug treatment （from 4% to 75% ）.The proportions of bone density and bone metabolism testing also rose markedly，positively impacting long-termpatientoutcomes.CONCLUSIONSTheestablishmentofastandardized management model forantiosteoporosistreatmentfolowing fragityfracturesurgerybyresidentclinicalpharmacistshasenhancedclinicians’diagnosticand therapeuticcapabilitiesforosteoporosis，ensuresrationalmedicationuseinosteoporosispatients，anddemonstratessignificant potential for widespread adoption and application.
