A study on association between pain and depression in elderly chronic disease patients in China

【Abstract】ObjectiveTo explore associationbetweenpainand depressive symptoms in elderly people with chronic diseases,and to asssthe impact of pain on the risk of depressve symptoms.MethodsBased onthe2O20 China Health and Retirement Longitudinal Study(CHARLS)data,anempiricalanalysiswasconducted,andacross-sectional analysiswas conductedon7211 elderlypeople with chronic diseases who were aged 6Oandabove.The study population were divided into depression group ( n=3133 )and non-depression group( n=4078 )based on their depressive status,and pain levelswere assessed through questionnaires.Amultivariate logisticregression modelwasemployed to exploreasociation between pain levels and theriskof depresion in elderlypeople with chronic diseases,and sensitivityanalysis and stratifiedanalysis were conducted.ResultsThe primary outcome measures were the prevalence of depression and positive corelation between pain levelsand theriskof depresion.Theresultsof themultivariatelogisticregressionanalysisshowedthatpainwasariskfactor fortheoccurrence ofdepressivesymptoms inelderlypeoplewith chronic diseases.As the pain level increased,therisk of depresion was1.35,1.77,1.89,and2.09 times higher thanthatof the pain-free population,respectively.Theriskof depression increased with the severity of pain,and difference was statistically significant ( P<0.001 ). ConclusionFor elderlypeoplewithchronicdiseases,painlevelssignificantlyincreasetheriskofdepresion.Painmanagementshouldberegardedas animportantstrategyforimprovingthementalhealthofthispopulation.Futureresearchesneedtofurtherexplorethecausalcorelation betweenpainand depressivesymptoms,and assessthepotential impact of pain interventionson depresivesymptoms.
