Evidence-based practice of preoperative bowel preparation for colonoscopy in hospitalized children

Abstract Objective：Evidence-based practice based on the best evidence for preoperative bowel preparation for colonoscopy in children to explore its application.Methods：Guided by an evidence-based continuous quality improvement model that applies the best evidence to hospitalized children needing colonoscopy through four phases：evidence acquisition，current status review，evidence introduction，and outcome evaluation，an interrupted time-series design was used to compare the preoperative bowel preparation compliance rate，tolerance of taking bowel preparation medications，implementation of each review metric，and pediatric providers' knowledge related to preoperative bowel preparation between pre-evidence-based practice（from February to May 2023） and post-evidence-based practice（from July to October 2023） in children.Results：Bowel preparation pass rate after evidence-based practice increased from 55.26% before practice to 76.19%，a statistically significant difference（P<0.05），decreased in the incidence of nausea and vomiting among adverse reactions seen in terms of tolerance after evidence-based practice compared to before evidence-based practice（P<0.05），improved implementation rates for indicators 1，2，3，6，7 and 9 of the review indicators（P<0.05），the scores of pediatric healthcare professionals on the assessment of knowledge related to preoperative bowel preparation for colonoscopy increased from （60.60±5.74） to （86.35±3.04），which was a statistically significant difference（P<0.01）.Conclusion：Evidence-based practice based on the best evidence for preoperative bowel preparation for colonoscopy in children improves tolerance of bowel-preparation medications，reduces the incidence of major adverse effects，and improves the implementation of evidence-based knowledge related to bowel preparation by pediatric healthcare professionals，children，and families.
