Systematic review of case report of subcutaneous emphysema during laparoscopic surgery in general surgery

Abstract Objective：To systematically review the case report of subcutaneous emphysema during laparoscopic surgery in general surgery.Methods：Computer retrieval was conducted on case report of subcutaneous emphysema during laparoscopic surgery in general surgery from databases such as VIP，WanFang Database，CNKI，PubMed，Web of Science，and the Cochrane Library.The retrieval time was from the inception to April 1，2024.Cluster analysis was conducted using SPSS 27.0 to determine the disappearance time of subcutaneous emphysema.Results：A total of 20 articles were included，including 16 Chinese articles and 4 English articles，published from 1991 to 2023，and involving 20 patients.The clustering analysis results showed that the disappearance time of subcutaneous emphysema could be divided into two categories.One category had a disappearance time of 20 minutes to 30 hours，with 7 cases undergoing gallbladder surgery and 1 case undergoing gastric resection surgery；the other category had a disappearance time of  subcutaneous emphysema of 2 days to 4 days，with 3 cases underwent gallbladder surgery，3 cases underwent rectal surgery，2 cases underwent colon surgery，and 1 case underwent pancreaticoduodenectomy.Conclusion：Current evidence shows that subcutaneous emphysema during laparoscopic surgery in general surgery is influenced by multiple factors.Clinical evaluation and intraoperative observation of laparoscopic surgery patients should be strengthened，and timely treatment should be carried out to reduce the occurrence of subcutaneous emphysema during surgery.
