Abstract Objective：To search and obtain the relevant evidence of transitional nursing for patients with chronic heart failure，and to summarize the best evidence，to provide ideas and reference for clinical medical staff and relevant decision makers.Methods：All literature related to transitional nursing for patients with chronic heart failure，including guidelines，best practices，summary of evidence，expert consensus were retrieved from BMJ Best Practice，Up To Date，JBI Evidence Based Health Care Center in Australia，NICE，SIGN，NGC，AHA，HFSA，ACC，AHRQ，the Cochrane Library，PubMed，EMbase，CINAHL，CNKI，and WanFang Database.Two researchers independently evaluated the quality of the literature，extracted and summarized the best evidence.Results：A total of 15 articles were included，including 6 guidelines，3 systematic evaluations，4 evidence summaries，and 2 expert consensus.Based on the characteristics of transitional nursing，a total of 19 pieces of evidence were extracted and summarized.Conclusion：The evidence summary of transitional nursing plans is scientifically standardized.It is recommended that evidence application personnel should choose the best evidence based on the hospital′s medical level and cultural background，in order to standardize the clinical nursing practice of transitional nursing in patients with chronic heart failure，improve patients′ self-management ability，and reduce the readmission rate of patients with chronic heart failure.
