间歇性管饲法在脑卒中后吞咽障碍病人中应用效果的Meta分析

Abstract Objective：To evaluate the effects of intermittent tube feeding on swallowing function，nutritional status，complications，quality of life and mood in patients with dysphagia after stroke by Meta-analysis.Methods：CNKI，WanFang Database，VIP，CBM，PubMed，EMbase，Web of Science，the Cochrane Library and MedLine were searched.The search period was from the establishment of each database to June 6，2022.Literature search，literature screening，data extraction，literature quality evaluation and bias risk assessment were conducted independently by two researchers.RevMan 5.4 software provided by the Cochrane Collaboration network was used for Meta-analysis.Results：A total of 15 articles were included，involving 1 208 patients，including 605 patients in the experimental group and 603 patients in the control group.Meta-analysis results showed that intermittent tube feeding can improve patient swallowing function［RR＝1.24，95％CI（1.15，1.32），P＜0.000 01］，reduce the incidence of associated pneumonia［RR＝0.33，95％CI（0.23，0.48），P＜0.000 01］ and injury rate of oropharyngeal mucosa［RR＝0.13，95％CI（0.04，0.42），P＝0.000 7］，increase serum albumin content［MD＝3.39，95％CI（2.43，4.35），P＜0.000 01］，hemoglobin content［MD＝5.04，95％CI（3.81，6.28），P＜0.000 01］，serum prealbumin content［MD＝23.27，95％CI（16.69，29.85），P＜0.000 01］，and body mass index［MD＝2.61，95％CI（2.16，3.05），P＜0.000 01］，improve patient quality of life［MD＝16.29，95％CI（11.53，22.30），P＜0.000 01］and comfort［MD＝0.80，95％CI（0.51，1.09），P＜0.000 01］，and improve patient anxiety［SMD＝－0.51，95％CI（－0.85，－0.17），P＝0.003］ and depression［SMD＝－0.37，95％CI（－0.70，－0.03），P＝0.03］.Conclusion：Existing evidence shows that，compared with indenting-in nasogastric tube，intermittent tube feeding can better improve the swallowing function of patients，improve the level of nutritional support，reduce the incidence of associated pneumonia and oropharyngeal mucosa injury，improve the quality of life and comfort of patients，improve patients′ depression and anxiety，and help patients better return to family and society.
