我院患者药物治疗管理的探索与实践

摘 要 本文介绍我院药物治疗管理工作情况，以及我院与社区医院开展联合药学门诊，参与慢性病患者药物治疗管理的工作模式与实践，总结取得的成效，并对未来发展提出设想。药物治疗管理的服务形式可以是多元化的，借助信息化工具能够提升管理水平，将服务由院内延伸至院外，让更多的患者得到高质量的专业药学服务。

Exploration and practice of medication therapy management for patients in our hospital

ABSTRACT The work of medication therapy management （MTM） in our hospital and the working mode and practice of our hospital and community pharmacists to carry out joint outpatient clinics and participate in the MTM for patients with chronic diseases were introduced， the results achieved were summarized， and strategies for future development were proposed. The service form of MTM can be diversified， and the management level can be improved and the service can be extended from in-hospital to outof-hospital with the help of information technology， so that more patients can get high-quality professional pharmaceutical services.

药物治疗管理（medication therapy management， MTM）是指具有药学专业技术优势的药师为患者提供用药教育、咨询指导等一系列专业化服务，从而提高患者用药依从性、预防用药错误，最终培训患者进行自我用药管理，以提高治疗效果[1]。（剩余5264字）
