社区老年人预立医疗照护计划研究进展

摘要  从社区视角出发，综述了预立医疗照护计划的相关概念、社区开展预立医疗照护计划的必要性，并对社区老年人预立医疗照护计划实施现状、干预模式进行分析，以期为我国社区预立医疗照护计划的科学实施提供理论依据。

Abstract    From the perspective of the community，this paper reviewed the relevant concepts of the advance care planning，the necessity of conducting the advance care planning，and analyzes the implementation status and intervention mode of the advance care planning for the elderly in community，in order to provide theoretical basis for the scientific implementation of the advance care planning in Chinese community.
