ABSTRACTObjectiveToevaluate the leftatrial structureand function changes in patients withhyperthyroidism by four-dimensionalautomaticlefttrialquantificationtechnology(4DAutoLAQ),andtoexploreitsclinicalvalue.MethodsFifty ninepatients withhyperthyroidism(casegroup)and41 healthy individuals(control group)during thesame periodwere included inour hospital.Conventional echocardiographywas used to measure leftatrial anteroposteriordiameter in end-systole(LADs), leftventricularend-diastolicdiameter(LVDd),leftventricularejectionfraction(LVEF),earlyandlatediastolicpeakbloodflow velocitiesacrossthemitral valve(E,A),earlyandlatediastolicmitralannularlateral wallmotionvelocities(e’,a’),and E/e'.4DAutoLAQwasusedtoobtainleftatrial maximumvolume(LAVmax),leftatrialminimumvolume(LAVmin),leftatrial pre-systolic volume(LAVpreA),leftatrialmaximumvolume index(LAVImax),leftatrial emptying volume(LAEV),leftatrial emptyingfractio(LAEF),leftatriallongiudinalandcicumferentialtraindurigthresevoirpase(LASrASrc)ral longitudinalandcicumferentialstraiduringtheonduitphase(LAScd,Acd-c),leftatriallongitudinalandciuertial strainduringthecontractionphase(LASct,LASct-c),andtheleftatrialstifessidex(LASI)wascalculated.Thediffrencesin above parameters between the two groupswere compared.The corelations between 4D Auto LAQ parametersand free triodothyronine(FT3),freethyroxine(FT4),andthyroid-stimulating hormone(TSH)inallstudysubjectswereanalyzed. ResultsConventional echocardiographyresultsshowedthatcomparedwiththecontrol group,thecase grouphad significantly increased LADs,LVDd,A,and a’(all P <0.05),while there were no significant diffrences in LVEF,E,e',andE/e'.4D Auto LAQ results showedthatcomparedwiththecontrol group,thecasegrouphadsignificantlyincreasedLAVmin,LAVpreA,LAVImax, andLASI,and significantlydecreasedLAEF,LASr,LAScd,LASct,LASr- ∇⋅c ,and LASct-c(all P <0.05),while therewere no significantdrencsinLVax,AEVd-celaioalisoedatLVinAEF,ASdt, LASr-c,LASct-c,and LASI were allcorrelated with FT3,FT4,and TSH in all study subjects (all P <0.05).Conclusion4D AutoLAQcanquantitativelyasssstheleftatrial structureand function changes inpatients withhyperthyroidism,offeringa reference for the precise clinical diagnosis of left atrial dysfunction and demonstrating certain value.
